INSURANCE & PAYMENT

At CARE-LA, we are committed to providing highly individualized, evidence-based mental health care for children, adolescents, adults, and families. In order to offer the highest quality treatment without the limitations often imposed by insurance companies, CARE-LA is an out-of-network provider and does not bill insurance directly. This allows our clinicians to focus on delivering personalized care based on clinical needs rather than insurance restrictions, session limits, or predetermined treatment requirements.

Although CARE-LA does not participate directly with insurance plans, many of our clients receive partial reimbursement for therapy services by using their out-of-network mental health benefits. After each session, we provide a detailed superbill (medical invoice) that includes all of the information your insurance company typically requires to process reimbursement, including CPT codes, diagnosis codes (when appropriate), our Tax ID number, and National Provider Identifier (NPI). Families submit these superbills directly to their insurance company, and reimbursement is determined by the terms of their individual health plan.

Because every insurance policy is different, we encourage families to contact their insurance provider before beginning treatment to learn about their specific out-of-network mental health benefits. Our administrative team is always happy to answer general questions about our billing process and help you understand what information you'll need to submit your claims.

Insurance Reimbursement

Many PPO and other health insurance plans offer reimbursement for services received from licensed out-of-network mental health providers. While reimbursement amounts vary by insurance carrier and individual policy, many CARE-LA families successfully receive reimbursement after submitting their superbills.

The CPT codes we most commonly use include:

​

  • 90791 – Diagnostic Evaluation

  • 90834 – Individual Psychotherapy (45–50 minutes)

  • 90837 – Individual Psychotherapy (53–60 minutes)

​

Your superbill will include the necessary billing information, including CPT codes, diagnosis codes (when clinically appropriate), CARE-LA's Tax ID number, and NPI number.

 

Please note that CARE-LA does not submit claims directly to insurance companies and cannot guarantee reimbursement, as all reimbursement decisions are made solely by your insurance provider.

Questions to Ask Your Insurance Company

Before your first appointment, we recommend contacting your insurance company and asking questions such as:

  • Does my plan include out-of-network mental health benefits?

  • What percentage of each therapy session is reimbursed?

  • Is there an annual deductible, and has it been met?

  • How many outpatient mental health sessions are covered each year?

  • Is preauthorization required before beginning therapy?

  • Are there any limits on reimbursement for psychological services?

  • How do I submit an out-of-network superbill?

  • How long does reimbursement typically take?

  • Is telehealth covered under my out-of-network benefits?

Understanding your benefits ahead of time can help you estimate your out-of-pocket costs and avoid unexpected expenses.

Payment

Payment is due at the time services are provided. CARE-LA's preferred payment method is ACH (electronic bank transfer/e-check), which does not incur any additional processing fees. We also accept major credit cards, personal checks, and cash for your convenience. Please note that all credit card payments are subject to a 3% processing surcharge.

 

Cancellation Policy

Because appointment times are reserved specifically for each client, we ask that cancellations be made at least 24 hours in advance. Appointments canceled with less than 24 hours' notice, or missed appointments, are subject to the full session fee. Whenever possible, we will make every effort to reschedule your appointment within the same week.

Does CARE-LA accept insurance?

CARE-LA is an out-of-network mental health practice and does not bill insurance companies directly. Many of our clients, however, receive reimbursement through their out-of-network mental health benefits after submitting the superbills we provide.

What is an out-of-network provider?

An out-of-network provider is a licensed healthcare professional or practice that does not have a contract with your insurance company. Instead of billing your insurance directly, you pay CARE-LA at the time of service and may then submit your superbill to your insurance company for possible reimbursement based on your individual benefits.

What is a superbill?

A superbill is a detailed medical invoice used to request reimbursement from your insurance company. CARE-LA's superbills include the information typically required for claims processing, such as CPT codes, diagnosis codes (when appropriate), Tax ID number, provider NPI, dates of service, and fees paid.

How do I know if my insurance will reimburse therapy?

The easiest way to find out is to contact your insurance company directly and ask whether your plan includes out-of-network mental health benefits. Because every policy is different, reimbursement amounts and eligibility vary by plan.

How much reimbursement can I expect?

Reimbursement depends entirely on your insurance plan. Some families receive little or no reimbursement, while others receive a significant portion of each session fee back after submitting their claims.

Why doesn't CARE-LA bill insurance directly?

Remaining out of network allows our clinicians to focus on providing individualized, evidence-based care without the restrictions that insurance companies may place on treatment, such as session limits, required diagnoses, or authorization requirements. This allows treatment decisions to be guided by your family's needs rather than insurance policies.

What information will CARE-LA provide for insurance reimbursement?

We provide a detailed superbill that typically includes the provider's name, Tax ID number, National Provider Identifier (NPI), diagnosis code (when appropriate), CPT billing code, date of service, and fees paid.

What forms of payment does CARE-LA accept?

CARE-LA accepts ACH (electronic bank transfer/e-check), major credit cards, personal checks, and cash. Payment is due at the time services are rendered.
